**Private pay and co-payments are due at date of service. Any outstanding balances will result in a hold being placed on services until payment is made. Payment plans can be arranged with our office manager. In the case of prolonged or non-payment from insurance, you will maintain responsibility of all charges and be reimbursed by the company in case of insurance payment.

FREQUENTLY ASKED QUESTIONS
DOES MY CHILD NEED AN EVALUATION BEFORE STARTING SERVICES?
If your child has received an occupational therapy or physical therapy evaluation within the last 5 months, a reassessment is not necessary if you are able to provide a complete copy of the assessment and any progress summaries completed since the initial evaluation. If it has been greater than 6 months or your child has never had an assessment completed, a comprehensive evaluation is required before services can begin.
WHAT SHOULD I BRING TO MY CHILDS EVALUATION?
Detailed medical history/documentation and any previous testing/treatment documentation including speech-language, PT, OT assessments, progress reports, developmental testing, psychological testing, IEP, IFSP, etc.
HOW LONG ARE THE SESSIONS?
Evaluation: 1-2 hours
OT/PT Sessions: 45-60 minutes
WILL MY CHILD SEE THE SAME THERAPIST EACH SESSION?
Here at SMSS we believe in a collaborative approach meaning your child will have their main treating therapist but may be seen by other therapists if they are receiving more than one service and/or receiving treatment more than once a week. This allows for increased collaboration to determine the best possible treatment plan and approaches for treating your child. This also allows for increased opportunities for makeup sessions when needed. SMSS and SCSS are a team and we believe that by working together we can provided unparalleled services to you and your child.

ATTENDANCE POLICY
No Shows and Cancellations
SCSS/SMSS strives to provide each patient with the highest quality of care while attempting to accommodate your schedule. We provide reserved time slots for each patient with a specific therapist to minimize your waiting and assure continuity of treatment. Consistent attendance of the treatment plan is paramount to your child’s progress. We understand that an emergency may occur in a rare instance. Our clinicians will make every effort and attempt to provide services to your child if time allows. Last minute cancellations and patient no-shows decrease our ability to accommodate the scheduling needs of the other patients and negatively impact clinician productivity. We must ask for your full cooperation with the following policy:
Repeated failure to comply with this ATTENDANCE POLICY will result in your name being placed on a “Schedule Based on Availability” list which requires you to call for an open appointment on each day you would like to receive therapy. We will do everything possible to accommodate you. This policy is necessary for the benefit of all our patients, in order to provide high quality treatment and service to everyone. The entire staff at SCSS/SMSS appreciates your adherence with this policy.
Rescheduling Sessions
SCSS/SMSS will do everything possible to schedule a makeup session as promptly as possible and may find it necessary to schedule you with another therapist. If your therapist takes a day off or goes on vacation, another therapist may fill in so that minimal changes can be made to your weekly therapy appointments. If you choose not to accept treatment from another therapist, you must inform your clinician that you want to cancel your session and adhere to the guidelines of the cancelation policy. Your session will not be rescheduled an additional time if you choose not to accept the makeup sessions provided.
